I accept Premera, Regence and Lifewise. I am otherwise out of network with insurance providers.

Some insurance companies give out-of-network reimbursements for mental health services. If you have spoken with your insurance carrier and wish to seek this type of reimbursement, I would be happy to provide you with a super bill (i.e. insurance reimbursement form with services listed) upon request.

Payments are due at the time of services. I accept all major credit, debit and HSA/FSA cards. For reimbursement from your insurance carrier, I will provide you with a super bill.
